Advance Care Planning

Advance Care Planning includes completing documentation that outlines your wishes for healthcare in case you are unable to make decisions for yourself, due to an accident, illness, or other reason. I am a certified Honoring Choices advance care planning facilitator.

Individual, couple, and family sessions are available to:

Discuss future healthcare decisions (CPR, intubation, dialysis, etc.)
Appoint a healthcare agent (who would make decisions for you when you are unable), and
Complete an Advance Directive document outlining future healthcare decisions that could be placed in an individual's medical file

A common misconception about advance care planning is that it is something that only older folks need to do. The thing is, you never know when an accident may happen, leaving a you unable to make decisions regarding you care. Advance care planning is for individuals of all ages. You can complete an advance care directive at any age, and change the details of it at any time.
